Chronic Disease Plan of Care:
This is a one time 1 hour virtual appointment to assist a non-member with the development of a plan of care to address chronic disease(s). This will include a 1 time ordering and interpretation of lab or radiology work-up and a 30 minute follow up to review diagnostic results i.e. labs, x-rays etc..
