Annual Physicals
This service includes a head to toe exam annual adult physical exam and child wellness exam. This will also include annual sports physical for youth. This exam will also include review of medications, health concerns, yearly labs ( if done prior to exam), appropriate screenings or referrals for screenings, and development of overall health plan of care for the upcoming year.
CHronic Disease Management
Chronic Disease Management will be executed with evidence based standard of care. Assessment will be done through laboratory and diagnostic order and review. An overall health impact assessment on patient is done per guidelines and as necessary. Care and management is individualized and plan of care is patient driven.
Acute
illness
Acute Illness treatment is individualized based on disease or illness. These can include same day or need to be seen within 72 hours, again depending on illness. Acute illnesses are things such as upper respiratory illness, asthma attacks, cuts, broken bones, headaches, stomach pain, pink eye, etc.
Hormone Replacement or Management
This service is not just about prescribing estrogen or testosterone, but it is truly assessing your symptoms and appropriately prescribing the hormones that will best relieve your symptoms safely. These appointments are every 2-3 months to include blood work prior to you visit. Most hormones are compounded and sent to the patient’s home.
Woman Sexual Dysfunction
This is a service that assess and treats sexual dysfunction including painful intercourse and low sexual desire in women. These services may include hormone replacement, but the focus is on sexual health and maintaining the health of the female sexual organs i.e. pelvic floor muscles, vagina, clitoris, etc.things such as upper respiratory illness, asthma attacks, cuts, broken bones, headaches, stomach pain, pink eye, etc.
Weight Management
This is an individualized plan that looks at biophysical and behavior as it relates to obesity. It is believed that obesity is a chronic illness and is not solved by a quick fad diet or pills, but is corrected with healthy lifestyle changes that will carry you through to a healthy energetic life. The provider works with the patient to set realistic goals. In other words not everyone is meant to be a size 2 or even a size 14. Genetics has a lot to do with weight as does lifestyle behaviors. A multi-facet of tools are used including, but not limited to pharmacotherapy.ll best relieve your symptoms safely. These appointments are every 2-3 months to include blood work prior to you visit. - This plan was designed for Families. This Plan includes service for a Family of 5 with children under 26.
- Families of 5 of more, we have a monthly Plan of $250.
Membership Includes
An initial home visit to establish care and set health goals.
Membership includes a yearly home visit for your annual physical exam.
Office visits can be used for PAP smears if desired.
Chronic illness memberships include 2 home visits.
One every 6 months for health care maintenance and blood work.
